Erectile dysfunction (ED) is the ongoing difficulty in achieving or maintaining an erection firm enough for satisfactory sexual activity.
An erection is a complex process involving blood vessels, nerves, hormones and psychological stimulation. When a man becomes sexually aroused, signals from the brain cause blood vessels in the penis to relax and widen. This allows blood to flow in and create firmness. If blood flow is reduced, nerve signalling is disrupted, or anxiety interferes with arousal, the erection may be weaker or difficult to sustain.
It is important to understand that ED is a medical condition. It is not a reflection of masculinity, virility, or desire. In fact, many men with ED still have normal sexual desire but struggle with the physical response. Seeking treatment is a positive step towards improving health and quality of life.
Erectile dysfunction becomes more common with age, but it is not limited to older men.
Research suggests that more than half of men between 40 and 70 will experience some degree of erectile difficulty. Younger men can also develop ED, particularly when stress, anxiety, lack of sleep or lifestyle factors are involved. Despite how common it is, many men delay seeking help for years. There is rarely a single cause. ED usually develops due to a combination of physical and psychological factors.

Certain prescription medications can affect erections. These include some antidepressants, blood pressure medications and treatments for prostate conditions.
Importantly, erectile dysfunction can sometimes be an early sign of underlying cardiovascular disease. The blood vessels supplying the penis are smaller than those supplying the heart. As a result, they may show symptoms of narrowing earlier.

PDE5 inhibitors are safe for most men when prescribed appropriately. However, they are not suitable for everyone. They must never be taken with nitrates, commonly prescribed for angina. The combination can cause a serious drop in blood pressure.
Caution is also required in men with:
Most side effects are mild and temporary. They can include headache, facial flushing, nasal congestion, indigestion and dizziness. Tadalafil may occasionally cause muscle or back discomfort.
Rare but serious side effects include sudden vision changes, hearing loss, or an erection lasting longer than four hours. Urgent medical attention is required if these occur.
